Companies House Forms Guide
A reference guide to the most common forms you will encounter as a UK company director — what each form does, when to use it, and where to file it.
Confirmation Statement
When to use: Filed at least once a year to confirm that the information Companies House holds about your company is correct and up to date.
The confirmation statement replaced the annual return in 2016. You must file it within 14 days of your confirmation date. It confirms details including registered office, directors, shareholders, and SIC codes.
Change of Director's Details
When to use: Used to notify Companies House of a change to an existing director's details — including name, service address, or residential address.
This form must be filed within 14 days of the change occurring. Most changes can be made quickly via WebFiling at Companies House.
Change of Registered Office Address
When to use: Used to notify Companies House that your company's registered office address has changed.
The new address must be in the same part of the UK (England & Wales, Scotland, or Northern Ireland) as stated in your articles of association. The change takes effect as soon as it is accepted by Companies House.
Appointment of Director
When to use: Used to notify Companies House when a new director is appointed to the company.
You will need the new director's full name, service address, date of birth, nationality, and occupation. The director must also complete a consent to act as director.
Termination of Director's Appointment
When to use: Filed to notify Companies House that a director has resigned or been removed from their position.
You must provide the director's name and the date their appointment ended. This is typically straightforward and can be filed online.
Striking Off Application
When to use: Used to apply to strike off (dissolve) a company that is no longer needed and has ceased trading.
Before filing DS01, the company must have stopped trading for at least three months, have no outstanding legal proceedings, and have no undischarged obligations to HMRC. All directors must agree to the application.
Where to file
All standard Companies House forms can be filed online via the Companies House WebFiling service at find-and-update.company-information.service.gov.uk. Paper filing is also available but takes longer to process.
Most forms can be filed free of charge online. Some forms — such as DS01 — carry a fee. Check the Companies House guidance for current filing fees.
